Stress Fractures

Dear Runner, Thank you for taking the time to open this email, I would be very grateful if you take the time to fill in this questionnaire. Below is a brief description of what the project is about and some instructions on completing the questionnaire. Yours, James Tullie PROJECT BACKGROUND I am a 4th year medical student at Edinburgh University. As part of our 4th year course we undertake a research project over 14 weeks in an area of our choice. I am a keen runner and orienteer, and over the past few years have seen several of my friends and training partners develop stress fractures. This led to me becoming interested in the subject and as a result coming up with the idea for this project. My intention is to find information from runners who have had a stress fracture, such as the site, age they got it etc. I would like to get information from as many runners in Scotland as possible and find out if there are any trends in the data. By the end of the project I would like to be able to advise runners knowledgeably about stress fractures and hopefully how to prevent them. INSTRUCTIONS for COMPLETING and RETURNING the QUESTIONNAIRE 1. Open the file and click on .Save As. 2. Keep the file name the same and make sure you save in .MY DOCUMENTS.. 3. Fill in the questionnaire as fully as possible. Put your answers in the boxes with a black border. Some have a drop down box with pre-set options (an arrow appears when you click on the box). Some require typing (note: some boxes are quite small but this does NOT limit the amount of text you can type in them). If you cannot answer a question or do not want to, just leave it blank. 4. Once you have filled in the questionnaire, click on SAVE. 5. Send an email to me at stressfracture@hotmail.co.uk with the completed questionnaire as an ATTATCHMENT. 6. If you have any questions or comments about the study feel free to add them in the email and I will do my best to respond to them. PLEASE NOTE: Any information entered on the questionnaire will be treated as confidential and will remain anonymous in any reports. Your email address will not be passed on to any other party.
